Course Evaluation Data Submission Form

Here you can submit course evaluation data. Please note:

1. Please submit data from one course at the time. 
2. To be able to submit the data you need the basic information (e.g. course title, the teacher responsible, number of registered students). Make sure you have all information available when you fill out the form. 
3. You will be asked to submit the data in an Excel file. Please make sure that the Excel file you are about to submit contains data from only one course.

Export the generated answer data to an Excel file and attach the Excel file with this form.Please keep in mind:

• The Excel file should contain answer data (and not be an automatically generated pdf-report).
• Do not change or remove anything from the Excel-file.
• Always name the file after the course code.
